Do I need photos of us for this one?
Yes - the trailer is made of them, so this format needs photos. Four or five is plenty: the one that makes them laugh, the one from the trip, the most recent one. Each carries a caption, so it plays as scenes rather than a gallery.
How do I get their answer?
The final act is the question card. They tap one of your answers or type their own, and it comes back to you as a reply on your Giftable - along with anything else you asked, like what time to pick them up.
